You may or may not have filled out the two forms that I currently use for my practice. Please fill out these forms if you have not filled them out in the past 6 months. I will check to see if you have before we meet. They are good to re-read, even if you have filled them out in the past. If you don’t have time now, please bookmark, email or text yourself this page so you can check the forms later and fill them out if you have not in awhile.
I look forward to working with you on a monthly basis. If you need to reschedule your appointment, you are able to do one at a time. Please reschedule for the same month to stay on schedule for your own regular self-care!
